Creating a Virtual Care Program For Complex Populations

Think Tank - 11:15 am - 11:35 am

While telehealth, remote monitoring and other virtual health modalities have seen significant growth in recent years, the COVID-19 pandemic has supercharged that growth. Providers and patients alike see it as a way to deliver necessary care while helping to protect the safety of both sides. Nevertheless, creating a virtual health program for populations with complex needs requires organizations to take a more flexible and expansive approach to meet patients where they are. This conversation will explore best practices and challenges from the lens of those on the front lines of building such programs.

Takeaways

  1. What are the various modalities available to deploy virtual care, and the benefits and challenges of each
  2. What are the needs of individuals with complex medical and behavioral issues, and how they can be addressed through virtual care options?
  3. How should social determinants be taken into account when deploying a virtual care program?
